Appointment and callback requests
This website collects your name, email address, optional phone number (required for benefits-review callbacks), preferred callback time, and selected service or package. The practice uses these details to contact you and arrange the next step. Submitting a request does not book an appointment.
Please do not submit diagnoses, medical records, insurance identification numbers, or other private health information through this form. It is an enquiry form, not a patient portal.
Practice updates
If you sign up for practice updates, your email address and consent are stored separately from appointment requests and shared with the practice to manage its updates list. Contact the practice to stop receiving updates.
How requests are handled
Requests are stored in the website’s separate database and forwarded to the practice at pt@advancedbalancestudio.com. TailorTech hosts and maintains the website for the practice. Access is restricted to authorized people who support the website and handle enquiries. Information is kept as needed to manage your request, with backups used for recovery.
The website uses HTTPS to protect information in transit. Email has its own privacy limitations; please discuss medical information directly with the practice through its appropriate care channels.
Browser storage and technical information
The form temporarily remembers your draft in this browser session. The draft is cleared after a successful submission. Short-lived technical information is used to limit abusive submissions. Website server logs support security and operation.
Optional analytics
If you allow analytics, Google Analytics helps the practice understand website visits, call-button clicks and successful request submissions. Form fields and service-selection query values are not sent to analytics. You can decline or change your choice using Privacy settings. Your preference is stored in this browser. Google processes analytics under its own privacy policy.
Photographs
Practice, clinician, treatment and product photographs come from Advanced Balance’s published website and catalog. The coastal backdrop is the approved Laguna Beach photograph by Logan Voss on Unsplash. It represents Southern California coastal life, not the clinic location. Photographs do not establish patient outcomes.
Payments and external links
Pay Bill opens BalanceCollect. This website does not collect payment card information or process payments. BalanceCollect, map and social-media services have their own privacy policies.
